SCHOOL HEALTH REQUIREMENTS FOR 2018-2019
PRE K
Physical exam
Up to date immunizations - including proof of Pneumococcal vaccine
Proof of Lead screening (call to set up an appointment at 942-6961)
Certified birth certificate
KINDERGARTEN
Physical exam
Up to date immunizations – including 2 doses of Varicella vaccine
Dental exam (anytime between October 15, 2017 and May 15, 2019)
Proof of Lead screening if not previously completed
Eye exam or waiver
Certified birth certificate
SECOND GRADE
Dental exam (anytime between October 15, 2017 and May 15, 2019)
SIXTH GRADE
Physical exam
1 Tdap vaccine – 2 doses of Varicella vaccine – 3 doses of Hepatitis B vaccine
Meningococcal vaccine
Dental exam (anytime between October 15, 2017 and May 15, 2019)
NINTH GRADE
Physical exam
2 doses Varicella vaccine
12th GRADE
2nd Meningococcal vaccine
SPORT PHYSICALS
Must be on a current sport physical form & be on file before the start of practice.
Children entering grades K -12 in an Illinois school for the first time are required to have certified birth certificate and an eye exam or waiver.
Physicals must be documented on State of Illinois current form. Parents must complete top portion of form. The doctor must thoroughly complete their portion of form. If physical form is not completed thoroughly by parent and/or doctor it will not be acceptable and considered noncompliant with state law.
